FACILITY DOG:

Facility dogs work with professional who incorporate the dog into the care of their clients. These dogs are most frequently placed in hospitals (e.g., physical therapy departments) or schools with special needs or mainstream students.






You might be interested in a Facility Dog if...
  • You are a teacher who works with children with physical or emotional difficulties and believes a facility dog would help students focus and apply concepts.
  • You are an occupational therapist in a hospital or skilled nursing facility and think a facility dog would help motivate your clients to do their therapy.

1. Print form.
2. Mail completed form and $25 application fee to:

Note: Application will not be processed until all required forms and application fee are received.
